Homeward Bound has a mission to work with others to end the cycle of homelessness with a primary purpose to move chronically homeless individuals into service-enriched permanent housing. Homeward Bound’s two main programs, the A HOPE Day Center and Pathways to Permanent Housing program, assist the chronically homeless access permanent housing through specialized case management services. With 25 years experience working with this population, the agency has learned to adapt to their ever-changing needs, keeping abreast of the necessary services available in the community, developing strong relationships and building collaborations with other agencies, and creating programming based on national best practices models. Homeward Bound believes in and follows the tenets of the best practices model called Housing First/Housing Plus, assisting chronically homeless persons find, procure, and maintain permanent housing by providing intensive, in-home case management with a focus on issues that could prevent clients staying in housing. A HOPE and Pathways to Permanent Housing staff attempt to develop trusting relationships with homeless clients, understanding that in order to make lasting change, clients need to make their own choices regarding their addiction, mental health, and physical health issues.

The chronic homeless constitute about 36% of the total homeless population, yet cost 80% of the total spent on services. Once in the cycle of homelessness, they have trouble accessing mainstream services. Because of living on the streets, they struggle with obtaining the basics such as food and clothing, let alone maintaining sobriety or mental health. If they have the opportunity to move into permanent housing, they rarely maintain it because of a lack of necessary skills or falling into bad habits that caused their homelessness. The Chronic Homeless Housing Project provides chronically homeless individuals 2 dedicated case managers to work towards and maintain housing. The Outreach case manager assists clients to apply for housing opportunities, begin the process of long-term sustainability through employment or benefits, and helps them transition, once housed, to the Housing case manager who visits them regularly in their home. This project’s focus is to get those homeless clients that cost the community most access to housing quickly and give them the services to maintain that placement. This project will help 40 persons obtain permanent housing annually. To support the case management necessary to assist these individuals maintain housing, the project requires funding of $85,000.
